I really like the guy in my choir who acts as my conversation partner, and I’d love to ask him out on a date in Welsh. Wyt ti eisiau mynd ir… [date with me]? I’d love some help with the proper wording so I don’t make a complete donkey of myself asking.

Expanding on your northern use of eisiau I’d go with
Wyt ti eisiau mynd ar ddêt efo fi?
Pob lwc!

Diolch! I’m seeing him again tonight, so I’m gonna need that luck.

Or, just to give you a choice, “wyt ti’n ffansio diod?” - very colloquial, but solid date asking Welsh
Pob hwyl!

I’d also like to wish you (and him) good luck.

He turned me down but was impressed with the Welsh, so it’s not all bad, haha!

Absolutely, and what a great way to go about practising your Welsh! In fact, you’ve just inspired me to add a ‘dating’ theme to the Word of the Day thread…
